Overview
The role of the
Social Worker is to create an environment in the home that provides opportunity for the community members to enjoy the highest possible quality of life and provides psychosocial support to residents, families, and partners, which will enhance the quality of life for residents and will promote the health of the intentional community of home.
- Shift: Full-Time
- Know each resident and their family including a psychosocial profile of the resident and an understanding of the family members and dynamics
- Participates in care planning for resident needs
Assures that out of facility activities are available to residents.
- Understand and complete MDSs timely and accurately
- Lead the Grievance Committee, hearing any resident concerns and following up on them timely
