What You’ll Do

• As our Home Health Registered Nurse Clinical Manager, you’ll be the heartbeat of our clinical operations. Your days will be filled with purpose as you:
• Support and mentor a dedicated team of nurses and caregivers
• Ensure our patients receive safe, high-quality, personalized care
• Oversee clinical documentation, care plans, and regulatory compliance
• Collaborate with physicians, referral partners, and families
• Lead case conferences and guide clinical decision-making
• Help onboard, train, and inspire new team members
• Keep a close eye on quality measures, patient satisfaction, and outcomes
• This RN role blends leadership, clinical expertise, and relationship-building — perfect for someone who loves both people and process.

Requirements

What You Bring

• Active MI RN license
• 2–3 years of clinical experience in home health, hospice, or a related field is preferred.
• Previous leadership or supervisory experience
• Solid understanding of Medicare home health OASIS is a plus.
• Comfort with EMR systems and clinical workflows
• Experience working with physicians, referral partners, patient families, and interdisciplinary care teams
• Bachelor of Science in Nursing (BSN) is a plus.
• Experience with Kinnser is a plus.
• Ability to work in the field as needed
• Eligible to work in the United States
