SUMMARY:
Coordinate patient care and facilitate the achievement of optimal outcomes in relation to clinical care.
ESSENTIAL DUTIES & RESPONSIBILITIES:
Coordinate patient care through a continuum and facilitate the achievement of optimal outcomes in relation to clinical care, quality, and cost effectiveness.
Accountable for making patient appointments and follow up.
Assess patient needs telephonically and/or in person.
Ensure compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the medical treatment facility. Provides care within ethical and legal boundaries.
Prescribe and communicates treatment plans and patient teaching in accordance with established protocols. Collect and assesses significant patient history information and performs all necessary patient teaching.
Make referral appointments and arranges provider scheduling (in coordination) as appropriate. May be required to manage surgical schedules.
