Lake Sunapee VNA and Hospice is seeking an experienced Nurse Practitioner (NP) to join our Hospice program in a per diem capacity to provide direct patient care visits for routine provider visits, symptom management and Hospice Face to Face. On-call rotation required.

You're a Nurse Practitioner (NP) who values personalized, patient-centered care and meaningful connections with patients and families. Whether you're experienced in home health and hospice or interested in learning more about these specialties, we'd love to connect with you.
You Bring:
- Experience in hospice and/or end of life care, and sympathetic management
- Strong communication with demonstrated ability to collaborate with an interdisciplinary team
- Comfortability with educating and guiding patients, families, facilities and community in end-of-life discussions, and decision-making

Qualifications:
- Current NH Nurse Practitioner (NP) license
- Minimum 2 years of health care experience required
- Hospice, Palliative or end-of-life care experience is preferred
- Additional requirements include reliable transportation, valid driver’s license, automobile insurance, current physical exam, TB testing, and required vaccinations.
