Liquid Personnel is seeking an experienced Senior Occupational Therapist to join its client's Community OT Team, supporting Pathway 1 Reablement services in Wolverhampton. This is a short‑term opportunity focused on community‑based OT work to promote independence and reduce ongoing care needs.
What will your responsibilities be?
- Deliver community‑based occupational therapy interventions within the reablement pathway
- Complete functional assessments to support safe discharge and prevent hospital admissions
- Promote independence through reablement‑focused practice
- Review progress and recommend appropriate equipment, adaptations, and onward support
- Work collaboratively with reablement staff, social care, and partner agencies
- Ensure timely, person‑centred interventions aligned with Pathway 1 objectives
