Our client, based in Telford is seeking an experienced Adult Social Worker to join their Early Intervention & Prevention Team. This role sits at the front door of Adult Social Care, focusing on early support, prevention, and reducing long-term dependency on services. You will work in a strengths-based, community-led way to empower individuals to maintain independence and achieve positive outcomes within their own homes and communities.

This is an excellent opportunity for a confident practitioner who is passionate about asset-based practice, proactive intervention, and managing complex cases, while balancing risk and promoting independence.

What will your responsibilities be?

Undertake Care Act 2014 assessments, reviews, and support planning using a strengths-based approach
Deliver early intervention to reduce, delay, or prevent long-term care needs
Manage a diverse caseload including adults with complex needs
Complete Mental Capacity Assessments (MCA) and contribute to Best Interest decisions
Lead and support safeguarding activity including Section 42 enquiries
Develop person-centred, asset-based care and support plans
