Social Worker - Community Team

Liquid Personnel is recruiting Social Worker for its client's Community Team at Slough.

This is an excellent opportunity for a dedicated practitioner to work with adults and their careers, delivering strengths-based, person-centred social work interventions that promote independence, wellbeing, and positive outcomes.

The successful candidate will hold a caseload of adults and/or their careers, undertaking assessments, care and support planning, reviews, safeguarding interventions, and risk management. Working within a community setting, you will apply a strengths-based approach to support individuals in achieving their desired outcomes whilst maintaining independence and maximising available community resources and support networks.

You will work collaboratively with individuals, families, carers, health professionals, providers, and partner agencies to ensure people receive appropriate and proportionate support in accordance with relevant legislation, departmental procedures, and best practice.

What will your responsibilities be?

Managing a caseload of adults and carers, undertaking assessments and interventions in accordance with statutory duties and best practice.

Completing strengths-based Care Act Assessments to identify eligible needs, personal outcomes, and appropriate support arrangements.
