Social Worker - Community Team

Liquid Personnel is recruiting Social Worker for its client's Community Team at Hull.

What will your responsibilities be?

Completed Care Act 2014 assessments, reviews and support planning for adults with a range of care and support needs, ensuring person-centred outcomes were achieved.

Undertook Mental Capacity Assessments and Best Interest decisions in line with the Mental Capacity Act 2005, promoting individuals' rights, choice and independence.

Managed duty responsibilities and worked collaboratively with health professionals, families and partner agencies through multi-disciplinary meetings to coordinate effective support and safeguard vulnerable adults.

Qualifications & Experience:

• To be successful in this role you must have, Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
