Social Worker – Learning Disabilities

Our client based in Walsall is seeking an experienced Social Worker to join their Learning Disabilities Team. This is an excellent opportunity for a dedicated practitioner to support adults with learning disabilities and their carers through strengths-based, person-centred social work interventions that promote independence, wellbeing, choice, and inclusion within the community.

Working within Adult Social Care, you will undertake Care Act assessments, care and support planning, reviews, safeguarding enquiries, and Mental Capacity Assessments. The successful candidate will work closely with individuals, families, carers, health professionals, and partner agencies to develop effective support arrangements and achieve positive outcomes for adults with learning disabilities.

The role requires a practitioner who can manage a varied caseload, work autonomously, and apply professional judgement within a legislative framework.

What will your responsibilities be?

In this role, you will, Undertaking strengths-based Care Act 2014 assessments for adults with learning disabilities and complex care and support needs.

Developing, co-producing, and reviewing person-centred Care and Support Plans that promote independence, self-determination, and community inclusion.
