Social Worker to join its client’s Mental Wellbeing Service team, based in Southampton

This is an excellent opportunity for a dedicated practitioner to work within a specialist Mental Health and Substance Use service, supporting adults to achieve positive outcomes, improve wellbeing, and maintain independence within their communities.

The successful candidate will work within a multidisciplinary environment alongside social workers, mental health practitioners, health professionals, and partner agencies to provide person-centred, strengths-based interventions. The role focuses on supporting adults with mental health and substance use needs through assessment, support planning, safeguarding, hospital discharge work, and access to community-based services.

This role requires a confident practitioner with strong knowledge of Adult Social Care legislation, mental health services, risk management, and partnership working, alongside a commitment to promoting recovery, independence, and social inclusion.

Supporting adults experiencing mental health and substance use difficulties to access appropriate services, support networks, and community resources.
