Social Worker - Preparation for Adulthood

Liquid Personnel is recruiting three experienced Social Workers for its client's Preparation for Adulthood (PFA) Team in Barnsley. (Competitive Hourly Rate)

This is an exciting opportunity to support young people aged 14-25 as they transition into adulthood, helping them maximise independence, achieve positive outcomes, and access the support they need for the future.

Working across both Children's and Adult Social Care, you'll play a key role in delivering strengths-based assessments, transition planning, and person-centred support for young people with a range of additional needs.

What will your responsibilities be?

Support young people aged 14-25 through transition into adulthood.

Complete assessments, reviews, and support plans using a strengths-based approach.

Work across both Children's and Adult Social Care legislation.

Promote independence, choice, and positive long-term outcomes.

Contribute to Education, Health and Care Plans (EHCPs) and transition planning.

Work closely with young people, families, carers, education providers, health professionals, and partner agencies.

Support young people with SEND and complex needs.
