Social Worker – West Locality Team

Liquid Personnel is recruiting an experienced Social Worker for its client's West Locality Team based in Walsall. This is an exciting opportunity to make a meaningful difference in the lives of adults with care and support needs by delivering strengths-based, person-centred interventions that promote independence, wellbeing, and positive outcomes.

As part of a supportive and collaborative team, you will work with adults, carers, families, and partner agencies to assess needs, manage risk, develop care plans, and ensure individuals receive the right support at the right time. This role is ideal for a confident practitioner who is passionate about empowering people to achieve greater independence while safeguarding those most at risk.

What will your responsibilities be?

Complete strengths-based assessments for adults and carers in line with the Care Act 2014.

Develop and review person-centred Care and Support Plans focused on achieving positive outcomes.

Manage a varied caseload, ensuring assessments, reviews, and interventions are completed within required timescales.
