Social Worker – Community Team

Liquid Personnel is recruiting an Adult's

Social Worker to join its client’s Community T eam, based in Barking and Dagenham.

What will your responsibilities be?

Manage a caseload of adults with varying levels of need, including complex cases depending on experience.

Complete Care Act assessments, reviews, and reassessments.

Develop and review person-centred support plans that promote independence and wellbeing.

Undertake safeguarding enquiries and manage risk effectively.

Complete Mental Capacity Assessments and apply the Mental Capacity Act where appropriate.

Support hospital discharges and work proactively to prevent unnecessary admissions.

Work collaboratively with individuals, families, health professionals, community partners, housing providers, voluntary organisations, and other partner agencies.

Provide information, advice, and guidance to adults and their carers.

Coordinate care and support to ensure individuals receive appropriate services and achieve positive outcomes.

Support vulnerable adults while promoting informed choice, independence, and wellbeing.

Maintain accurate, timely, and high-quality case records.

Contribute to service development and continuous improvement initiatives.

Support and supervise less experienced colleagues where appropriate.

Qualifications and Experience:

• To be successful in this role you must have; A recognised Social Work qualification
• Current Social Work England (SWE) registration
• Experience undertaking assessments, reviews, and safeguarding interventions within adult social care
• Good knowledge of the Mental Capacity Act and relevant legislation
• Strong multi-agency partnership working skills
• Excellent communication, organisational, and problem-solving abilities
• A compassionate, proactive, and person-centred approach to practice
