Social Worker - Hospital Discharge

Liquid Personnel is recruiting two Social Workers for its client’s Hospital Discharge Team in Stoke-on-Trent City. These roles are to backfill positions while existing team members are on secondment.

What will your responsibilities be?

• Conduct assessments to identify patients’ social, emotional, and practical support needs
• Develop safe and effective discharge plans in collaboration with the multidisciplinary team
• Provide crisis intervention, emotional support, and counselling to patients and families
• Advocate for patients to ensure access to appropriate services and community resources
• Coordinate referrals to external agencies, housing services, mental health teams, and safeguarding teams
• Support patients with complex needs, including safeguarding concerns, domestic abuse, and homelessness
• Facilitate communication between patients, families, and healthcare professionals
• Maintain accurate and up-to-date case notes and documentation
• Participate in multidisciplinary meetings and case reviews
• Promote patient wellbeing and independence through tailored care planning

Qualifications & Experience:

• To be successful in this role you must have, Professional Qualification: A recognised professional qualification in social work, such as an MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW (Certificate of Qualification in Social Work).
• Registration: Must be registered with Social Work England, the regulatory body for social workers in England.
• Experience: 2+ years post qualified experience as an adult social worker in a statutory setting.
• Knowledge of Legislation: Thorough understanding of relevant legislation, such as the Care Act 2014 and the Mental Capacity Act 2005.
• Skills: Strong assessment and analysis skills, as well as the ability to work effectively within a multidisciplinary team.
• Driving Licence: A full UK driving licence and access to a car are often required for client visits.
