Social Worker to join its client’s Hospital Discharge team, based in Lancashire.

What will your responsibilities be?

Provide statutory social work support — delivering professional social work interventions and fulfilling legal duties under adult social care legislation.

Promote independence and recovery — completing strengths‑based assessments and enabling individuals to achieve the best possible outcomes.

Coordinate safe hospital discharges — working closely with health partners to ensure timely, appropriate discharge planning and community support.

Qualifications and Experience:

• To be successful in this role you must have; Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
