Social Worker - Single Point of Access Team

Liquid Personnel is recruiting an Adult/ Children’s

Social Worker to join its client’s Single Point of Access team, based in Flintshire.

What will your responsibilities be?

Conduct safeguarding screenings, community care assessments, and risk assessments for adults, determining appropriate interventions and managing complex cases within the Single Point of Access service.

Manage an allocated caseload, developing, implementing, and reviewing outcome-focused care and support plans that promote independence, minimise risk, and improve wellbeing.

Work collaboratively with individuals, families, carers, and multi-agency partners to coordinate services, provide professional advice, maintain accurate records, and progress safeguarding and referral actions.

Qualifications and Experience:

• To be successful in this role you must have; Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
