Social Worker - Adult Mental Health Team

Liquid Personnel is recruiting Social Worker for its client's Adult Mental Health Team at Portsmouth.

This is an excellent opportunity for a dedicated practitioner to support adults experiencing mental health difficulties, promoting independence, wellbeing, and positive outcomes through strengths-based and person-centred social work practice.

The successful candidate will work with adults who have complex mental health needs, undertaking statutory assessments, safeguarding interventions, support planning, and reviews. Working collaboratively with health professionals, carers, families, housing services, and partner agencies, you will help individuals access the support they need to remain safe, independent, and connected to their communities.

This role requires a confident Social Worker who can manage a varied caseload, work autonomously, and respond effectively to changing priorities whilst maintaining a strong focus on recovery, risk management, and person-centred outcomes.

What will your responsibilities be?

Undertaking Care Act 2014 assessments to identify eligible needs and develop appropriate support arrangements.

Developing, implementing, and reviewing person-centred Care and Support Plans that promote independence, recovery, and wellbeing.

Managing a varied caseload of adults with mental health needs and complex social care requirements.

Completing Mental Capacity Assessments and contributing to Best Interest decision-making in accordance with the Mental Capacity Act 2005.

Undertaking safeguarding enquiries and interventions in line with statutory guidance and local safeguarding procedures.

Participating in the duty rota and responding to urgent referrals, safeguarding concerns, and crisis situations when required.

Working collaboratively with mental health professionals, community teams, carers, families, housing providers, and voluntary organisations.

Assessing and managing risk appropriately, balancing safety, choice, independence, and positive risk-taking.

Supporting adults and carers through periods of change, crisis, and transition, ensuring timely access to appropriate services and support.

Promoting strengths-based practice and supporting individuals to access community resources and informal support networks.

Maintaining accurate, timely, and legally compliant records, assessments, reports, and case documentation.

Contributing to multidisciplinary meetings, safeguarding discussions, reviews, and service development activities.

Qualifications & Experience:

• To be successful in this role you must have, Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
• Registration: Must be registered with Social Work England.
• Experience: Significant post-qualified experience within Adult Social Care or Adult Mental Health Services.
• Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, Mental Health Act 1983, and associated statutory guidance.
• Assessment Skills: Proven ability to undertake Care Act assessments, Mental Capacity Assessments, reviews, risk assessments, and support planning.
• Safeguarding Experience: Experience undertaking safeguarding enquiries and managing complex safeguarding concerns.
• Mental Health Experience: Experience supporting adults with complex mental health needs and working within multidisciplinary environments.
• Communication Skills: Excellent communication, report-writing, negotiation, and decision-making skills.
• Professional Practice: Commitment to strengths-based, person-centred practice and continuous professional development.
• IT Skills: Competent in the use of electronic case management systems and Microsoft Office applications.
