Social Worker – Integrated Discharge

Social Worker to join its client’s

Integrated Discharge team, based in Nottingham.

This is an excellent opportunity for a skilled practitioner to work within a fast-paced hospital discharge service, supporting adults with complex care and support needs to achieve safe, timely, and effective discharges from hospital.

The successful candidate will work as part of a multidisciplinary team alongside health professionals, community services, carers, families, and partner agencies to promote independence, wellbeing, and positive outcomes. Using strengths-based and person-centred approaches, you will undertake statutory assessments, coordinate support services, manage risk, and develop discharge plans that enable individuals to return safely to their communities with the appropriate support in place.

This role requires a proactive Social Worker who can work effectively under pressure, manage competing priorities, and support individuals through periods of transition while ensuring compliance with Adult Social Care legislation and best practice.
