Conduct initial and follow-up telehealth assessments on flagged patients; initiate/adjust plans of care; coordinate with referring and primary care providers—all via secure video/audio platform.
Data into action: Interpret trend-based insights from the Circadia platform, prioritize high-risk patients, and drive timely interventions to prevent decline and unnecessary ED visits.
Close the loop: Complete documentation/charts within the same day.
Tele-visit management: Efficiently complete 25+ tele visits per day for designated home health patients in the SoCal/OC area, focusing on new admissions, high-acuity flagged patients, and Circadia alerts.
Quality & product feedback: Surface insights to improve virtual workflows and tech integration in a home health setting.
Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.
How Success is Measured (KPIs): Visits: Target 25+ patient tele visits/day
Flagged Patients are addressed within 24 hours
Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.
Adoption & impact: Demonstrated workflow adherence across patient panels; trend toward fewer falls/hospitalizations and improved home health star ratings over time.
Must-Have Qualifications: Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
Eligible for Medicare participation; company bills on your behalf.
2+ years NP experience with complex adult populations in home health or post-acute settings; able to practice independently.
Comfortable conducting telehealth visits and navigating virtual care platforms.
Thrives in a dynamic, evolving startup environment.
English required.
Nice-to-Haves: Tech-savvy: Google Suite, multiple EMRs (HCHB experience a plus), Slack, AI transcription/tools.
Spanish preferred.
