Virtual Neurology Care - Nurse Practitioner (Remote) NH

Role Overview: Nurse Practitioner, Neurology and Memory Care

We are seeking a skilled and compassionate Nurse Practitioner who is licensed in New Hampshire to join our virtual care team. This is a remote, patient-facing clinical role; delivering care to Local Infusion patients and partnering closely with physicians and the broader care team to increase patient access and reduce time to treatment.

• • Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth
• • Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians including prescribing therapies
• • Partner with our neurologist(s) and broader care team to ensure continuity of care
• • Educate patients and families on diagnoses, treatment plans, and disease progression
• • Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements
• Key Accountabilities
• • Active, unrestricted NP license in New Hampshire
• ; certification in geriatrics, adult-gerontology, or family practice preferred
• • 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
• • Strong clinical judgement and clinical documentation skills
• • Comfortable managing a full patient panel independently with physician collaboration
• • Excellent communication skills with patients facing cognitive decline and their caregivers
