RN Hospice Admissions
This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services.
Position Responsibilities:
- · Coordinate and oversee all aspects of new patient admissions
- · Collaborate with patient care teams to support seamless communication and continuity of care
- · Educate patients and families regarding condition, needs, and care
- · Perform head-to-toe physical assessments
- · Complete all required documentation accurately and on time
Requirements
- · Current Registered Nurse license
- · Current driver's license
- · Current CPR certification
- · Two years of Registered Nurse experience in a clinical care setting
- · One year of full-time hospice experience
- · Reliable transportation and valid automobile liability insurance
