Overview
POSITION SUMMARY: The LPN Home Health collaborates with the RN Case Manager to develop and implement the home health plan of care. The LPN Home Health communicates with the patient’s provider and other members of the care team such as nursing, therapy, social work, home care aide, respiratory therapy, home medical equipment and home infusion services to facilitate and expedite the best possible patient outcomes.
