LSCHC and the Chum Center are collaborating on a grant from the Minnesota Department of Heath to improve access and delivery of coordinated, comprehensive medical, mental health services and promote housing stability for people with serious mental illness and/or co-occurring disorders who are experiencing homelessness. In addition to Chum, there will be collaboration with other community partners to support our community’s homeless population.
This individual understands the root causes of homelessness, has work or lived experience helping individuals to access healthcare and housing resources, has cultural competency and a willingness to learn about the cultural needs of people, thrives in an unpredictable work environment and shares our commitment to end homelessness through action and advocacy.
