MDS Nurse RN

Overview:

The MDS Coordinator (RN) is a key member of the interdisciplinary health care team responsible for coordinating the development, completion, accuracy, and transmission of resident assessments and person-centered care plans. This position ensures documentation and coding comply with federal and state regulations, support appropriate reimbursement, and reflect each resident’s needs, preferences, goals, and clinical status.

The MDS Coordinator serves as a resource to residents, families, and team members throughout the assessment and care-planning process.
