Job Summary
The MDS Coordinator (Minimum Data Set Coordinator) is the primary point of contact for the review and coordination of patient care plans to promote quality care and services to residents. This role works with other nursing department colleagues to ensure that patient care is coordinated and communicated effectively with the clinical team. MDS job coordinators determine the health status, care plans, and procedures for intake of residents according to state and federal standards.
The successful candidate will possess a strong understanding of healthcare regulations and have experience in clinical assessment and documentation.
Essential Duties and Job Responsibilities
Review resident care plans for appropriate goals, problems, approaches, and revisions based on nursing needs.
