(APP) is a Physician Assistant (PA), Nurse Practitioner (NP), or Advance Practice Registered Nurse (APRN). The APP works in accordance with a standard collaborative care arrangement with a physician or to provide primary care services collaboratively within a multidisciplinary health team. The APP is responsible for providing comprehensive patient-centered care in an integrated healthcare setting. This position is responsible for individual services to patients assigned by Crossroads Health in the Integrated Healthcare Center

Additionally, the Prescriber will:

Provide general comprehensive primary medical care services to individual patients across the life span in the office setting including behavioral health assessment, medication evaluation and medication management, and prescription of medication in compliance with Ohio state regulations using a trauma informed approach.
