- Full Time; Monday through Friday 8am - 5pm
- Springfield, MO and surrounding areas
- Pay Range: $60,000-70,000
The Comprehensive Disease Management (CDM)/Palliative Nurse Navigator helps patients navigate their healthcare journey by having a goals of care conversation, identifying their healthcare needs, recommending resources, communicating with patient's healthcare providers and educating on disease progression and treatment/care options available.
