Social Worker - Discharge to Assess Team

Liquid Personnel is recruiting Social Worker for its client's Discharge to Assess Team.

This is an excellent opportunity for a dedicated practitioner to support adults with complex care and support needs following hospital discharge, helping them maximise their independence, wellbeing, and choice through strengths-based and person-centred practice.

The successful candidate will work within a fast-paced multidisciplinary environment alongside health professionals, community services, carers, families, and voluntary sector partners. You will play a key role in assessing needs, facilitating safe discharges, promoting reablement opportunities, and ensuring individuals receive the appropriate support to achieve the best possible outcomes within their communities.

This role requires a confident and experienced Social Worker with excellent assessment skills, strong legislative knowledge, and the ability to manage complex decision-making whilst supporting individuals through periods of change and recovery.

What will your responsibilities be?

Undertaking Care Act 2014 assessments, reviews, reassessments, and support planning within the Discharge to Assess pathway.

Using a strengths-based and person-centred approach to support adults to maximise independence, choice, and wellbeing.

Completing complex assessments using a range of health and social care frameworks and contributing to Continuing Healthcare (CHC) processes where required.

Identifying opportunities for reablement, enablement, and preventative interventions to reduce long-term dependency on services.

Undertaking Mental Capacity Assessments, applying the two-stage test, and facilitating Best Interest decision-making in accordance with the Mental Capacity Act 2005.

Leading and completing safeguarding enquiries, including complex safeguarding concerns and whole-service safeguarding investigations where required.

Assessing and managing risk effectively, balancing individual choice with safety and positive risk-taking.

Working collaboratively with health professionals, multidisciplinary teams, community services, carers, families, and voluntary sector organisations to coordinate effective support arrangements.

Supporting safe and timely discharges from hospital and ensuring robust follow-up support is in place within community settings.

Undertaking work relating to the Court of Protection, including applications relating to deprivation of liberty in community settings where appropriate.

Preparing high-quality assessments, reports, support plans, and professional documentation in accordance with statutory and organisational requirements.

Maintaining accurate, timely, and reflective case records using electronic case management systems.

Contributing to service development, quality improvement initiatives, and the promotion of best practice across Adult Social Care services.

Qualifications & Experience:

• To be successful in this role you must have, Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
