Advanced Practice Provider (APRN/PA-C) – Geriatric Medicine at Concord Hospital | LAYIQ

Master's degree from an accredited APRN or Physician Assistant program
Current or eligible New Hampshire APRN or PA-C licensure
Current DEA registration
Minimum of two years of post-graduate clinical experience in geriatrics, primary care, home-based care, or nursing facility medicine
Experience providing care in home-based and/or nursing facility settings strongly preferred
Demonstrated interest in caring for older adults with complex medical needs
Strong clinical assessment, communication, and interdisciplinary collaboration skills
