Job Overview
The Linkage to Care Coordinator II is responsible for the seamless enrollment into medical care and ancillary services of all newly diagnosed HIV and Hepatitis C patients and patients transferring care to AltaMed . Th e Coordinator support s patients to reduce barriers to care by facilitat ing access to medications and linkage to ph arm a cy services, connect ing clien ts to Behavioral Health and substance abuse services, and linking clients with appropriate case management services and prevention counseling.
