Position Summary
The Community Health Worker is part of an interdisciplinary care team that assists in care coordination, informing patients on community resources, and improving population health. Responsibilities:
Performs cross continuum care activities, which may include coordinating care between primary care clinics, specialty care clinics, urgent care clinics, or other sites of care, as appropriate.
Engages in outbound communication to patients and keep them engaged in their care, set-up appointments, and prompt them to act.
