Family Bridge Community Health Worker

All employees of Bridgeport Hospital are part of the patients care team and contribute to the team approach of promoting access, continuous, comprehensive care and work to provide quality improvement in the care provided to their patients.

Under the supervision of the Program Director, provides community service navigation for Family Bridge families. Responsibilities may include engaging patients, and families and helping individuals navigate and access community services, and resources. The Community Health Worker will provide education and advocacy to assist individuals with accessing services.
