Cardiology Nurse Navigator

Overview

The nurse navigator functions as a clinical resource and patient advocate across the continuum of cardiovascular care, supporting patients receiving care in the outpatient cardiac clinic and hospital-based procedural/interventional areas including Cardiology, Cardiac Catheterization Laboratory, Electrophysiology (EP), Interventional Radiology (IR), and related outpatient and inpatient services.

The Nurse Navigator coordinates care for patients undergoing diagnostic and interventional cardiovascular procedures, promotes timely access to services, facilitates communication among multidisciplinary teams, and supports quality, safety, patient education, and continuity of care across the continuum of care. The nurse navigator assists in the development of educational materials and program monitoring to promote coordination of care and patient satisfaction.

The Nurse Navigator assists the clinical and leadership team with program development, expansion and successful achievement of programatic goals.
