Community Health Nurse

Overview

The Community Health Nurse is responsible for providing comprehensive nursing care to clients in a community-based setting. The duties of this role vary by location but includes conducting health assessments, administering medications, implementing physicians' orders, and coordinating care with onsite and community healthcare providers.

The Community Health Nurse monitors client health status, provides first aid and emergency response when needed, and educates clients and staff on medication management, disease prevention, and Narcan use. The position also involves developing and overseeing nursing care plans, ensuring a safe and hygienic environment, maintaining accurate clinical documentation and data reporting, and collaborating closely with program leadership and multidisciplinary staff to support client health, safety, and continuity of care.
