Now Hiring: Care Coordinator
Full Time | $18 to $21 per hour
Lebanon. MO
One of the most unsung heroes in any home care agency is the Care Coordinator. But at Phoenix Home Care & Hospice we sing praises from the mountaintops about our Care Coordinators. A truly great Care Coordinator is one of the most valuable, high-leverage roles on our team. As a Care Coordinator you are the Communication Hub, the Emotional Support Co-Worker, and the Air-Traffic Controller for our nurses and caregivers in flight.

What You Will Do
- Coordinate daily caregiver schedules based on client care plans and staff availability
- Support and communicate with caregivers working in the field
- Complete client openings in the field
- Respond to client and employee questions or concerns and assist with resolution
- Maintain accurate documentation, records, and reports
- Assist with quality initiatives, staff communication, and training support
- Provide professional and courteous communication with clients, families, and team members
What We Are Looking For
- Must be at least 18 years of age
- High school diploma or equivalent required
- Previous office or administrative experience required
- Strong communication and organizational skills
- Ability to multitask and stay organized in a fast-paced environment
- Professional, dependable, and team-oriented attitude
