Summary of Job Duties:
The Registered Nurse will promote wellness, improve outcomes, and support self-care within a defined population of patients. The RN will provide patients who have chronic conditions the support, education, and assistance in the prevention and/or maintenance of their disease and/or health and wellness state. Will work with the individual patient, their families, and providers of care to develop and implement individual health plans and provide support to the patient in self-management.
Coordinate care and/or follow-up for patients who receive care in outpatient facilities. Coordinate prescription refill requests and patient education in collaboration with physician staff. Commitment to patient outcomes results in high levels of performance despite workload changes.
In addition, the RN Coordinator plays a Central role in ensuring seamless care for patients with HHT and/or Vascular Anomalies. The Coordinator provides specialized, patient-centered nursing care and serves as the primary coordinator for patients receiving care through the HHT and Vascular Anomalies Program. The role coordinates diagnostic testing, specialty consultations, treatment plans, and ongoing communication between patients, providers, and multidisciplinary teams.
The RN provides patient and family education, triages clinical concerns, facilitates timely follow-up, and supports complex care coordination across the continuum. The position also supports HHT and Vascular Anomalies-related research activities, including patient screening, enrollment, data collection, and regulatory requirements. Strong clinical judgment, communication, organization, care coordination, and proficiency with the electronic health record are essential to the role.
Supports continued program growth through patient and family engagement, educational initiatives, community and provider outreach, Family Day, and other innovative programs that improve awareness, access, and the patient experience.
Qualifications:
Minimum Qualifications:
Graduate of an accredited Nursing Program with an Associate's Degree, Nursing Diploma, Bachelor's in Nursing, or Master's in Nursing; Valid RN License; Minimum of one (1) year of Registered Nursing experience.
Preferred Qualifications:
Baccalaureate Degree, RN Degree
Three (3) years of RN experience, with two (2) years of experience in a clinical environment
Current BLS certification or must successfully obtain certification within the first 30 days of employment

Collects and documents assessment data from patients and/or others per UAMS standard.
Develops patient's Plan of Care (POC) per department/unit standards utilizing assessment data.
Evaluates the progress of patients toward attainment of desired outcomes.
Administers medication following department/unit standards and Nurse Practice Act.
Verifies and evaluates care delivered to ensure it is consistent with patient and family centered care principles, practice standards and regulatory requirements.
Supports the integration of current evidence into nursing practices.
Promotes an environment conducive to positive educational experience for all students and staff.
Effectively collaborates and communicates with nursing staff at all levels, interdisciplinary teams, executive leaders, and other stakeholders.
Communicates care appropriately through documentation in the medical record.
Reports variances in care/ systems/ processes through the appropriate means.
Participates in recruitment/retention of staff.
Facilitates development and implementation of nursing practice standards that are evidence-based and in compliance with ANA standards.
May perform other duties as assigned.
