Social Worker – Disabilities

Liquid Personnel is recruiting an Adult’s

Social Worker to join its client’s Disabilities team, based in Powys.

What will your responsibilities be?

Managing a caseload of adults with physical disabilities, sensory impairments, learning disabilities, long-term health conditions, and complex care and support needs.

Undertaking strengths-based assessments in accordance with the Social Services and Well-being (Wales) Act 2014.

Developing, implementing, and reviewing person-centred Care and Support Plans that promote independence, wellbeing, and positive outcomes.

Working collaboratively with individuals, carers, families, advocacy services, health professionals, housing providers, and partner agencies.

Undertaking safeguarding enquiries and responding appropriately to concerns relating to abuse, neglect, exploitation, and self-neglect.

Completing Mental Capacity Assessments and contributing to Best Interest decision-making processes where required.

Assessing and managing complex risks whilst promoting positive risk-taking and individual choice.

Supporting adults through significant life changes, transitions, accommodation issues, and changes in support arrangements.

Coordinating services and support packages to ensure individuals receive timely and effective interventions.

Promoting access to community resources, direct payments, preventative services, and support networks that enhance independence.

Participating in multidisciplinary meetings and contributing to integrated care and support planning.

Preparing high-quality assessments, reviews, reports, and professional documentation.

Maintaining accurate, timely, and legally compliant case records in accordance with organisational requirements.

Qualifications and Experience:

• To be successful in this role you must have; Professional Qualification: Degree in Social Work, Diploma in Social Work, CQSW, or equivalent recognised Social Work qualification.
• Registration: Must be registered with Social Care Wales.
• Experience: Significant post-qualified experience within Adult Social Care services.
• Disabilities Experience: Experience supporting adults with physical disabilities, learning disabilities, sensory impairments, and complex care needs.
• Legislation Knowledge: Strong understanding of the Social Services and Well-being (Wales) Act 2014, Mental Capacity Act 2005, Human Rights legislation, and Adult Safeguarding procedures.
• Assessment Skills: Proven ability to undertake strengths-based assessments, reviews, care and support planning, and risk management interventions.
• Safeguarding Experience: Experience managing safeguarding concerns and undertaking safeguarding enquiries involving vulnerable adults.
• Multi-Agency Working: Strong experience working collaboratively with health professionals, housing providers, carers, advocacy organisations, and partner agencies.
• Communication Skills: Excellent communication, report-writing, assessment, analytical, and decision-making skills.
• Professional Practice: Commitment to strengths-based, person-centred practice with a focus on independence, wellbeing, and inclusion.
• Driving Licence: Full UK driving licence and access to a vehicle is desirable due to the community-based nature of the role.
