Social Worker - Adult Community Team

Liquid Personnel is recruiting Social Worker for its client's Adult Community Team at Manchester

What will your responsibilities be?

Manage a caseload of adults with a range of care and support needs, promoting independence, wellbeing and choice.

Undertake comprehensive strengths-based assessments of individuals’ care, support and social needs.

Complete Care Act assessments and reviews, determining eligibility and identifying appropriate support arrangements.

Develop, implement and review person-centred care and support plans in partnership with individuals, their families and carers.

Work collaboratively with individuals to identify their strengths, goals, networks and community resources.

Promote independence, prevention, recovery and least restrictive approaches wherever possible.

Assess and respond appropriately to safeguarding concerns, working in accordance with local safeguarding procedures and statutory responsibilities.

Undertake risk assessments and develop proportionate risk management plans in partnership with individuals and relevant professionals.

Support individuals to access appropriate services, resources, benefits, housing, employment, community activities and other sources of support.

Work collaboratively with health professionals, voluntary and community organisations, housing providers and other partner agencies to coordinate effective support.

Provide advice, information, advocacy and signposting to individuals, families and carers.

Work alongside carers to identify their needs and ensure their contribution is appropriately recognised within assessment and support planning.

Facilitate and coordinate multi-agency meetings, reviews and professional discussions where required.

Support individuals through changes in circumstances, including hospital discharge, transitions between services and changes in care arrangements.

Apply relevant legislation, policies and professional standards, including the Care Act, Mental Capacity Act, Human Rights Act and safeguarding legislation.

Promote equality, diversity, inclusion, dignity and respect in all aspects of practice.

Maintain accurate, comprehensive and timely case records in accordance with organisational and professional requirements.

Manage workload effectively, prioritising cases according to need, risk and urgency.

Work within professional boundaries while developing positive, respectful and effective relationships with individuals, families and partner agencies.

Participate in supervision, reflective practice, team meetings, training and continuing professional development.

Contribute to service development, quality improvement and the ongoing development of effective social work practice within the community team.

Qualifications & Experience:

• To be successful in this role you must have, Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
