Social Worker - Learning Disabilities

Liquid Personnel is recruiting an Adult’s

Social Worker to join its client’s Learning Disabilities team, based in Doncaster.

What will your responsibilities be?

Provide person-centred social work support to adults with learning disabilities, promoting independence, choice, rights and wellbeing.

Complete holistic assessments of care and support needs in line with relevant legislation and local authority procedures.

Develop, review and coordinate personalised care and support plans with individuals, families and partner agencies.

Safeguard adults at risk of abuse or neglect, responding appropriately to safeguarding concerns and following statutory procedures.

Work collaboratively with individuals, families, carers and multidisciplinary professionals to achieve positive outcomes.

Support people to access appropriate health, social care, housing, education, employment and community services.

Advocate for individuals and ensure their views, wishes and preferences are central to decision-making.

Undertake Mental Capacity Act assessments and contribute to best-interest decision-making where appropriate.

Apply relevant legislation, policies and professional standards to social work practice.

Support individuals through transitions and significant changes in their lives, including changes in accommodation, services or support arrangements.

Manage a varied caseload, maintaining accurate and timely records and completing statutory documentation.

Work proactively with partner organisations to develop creative and appropriate support solutions.

Promote inclusion, equality, dignity and the rights of people with learning disabilities.

Participate in reviews, case discussions, team meetings and other multi-agency forums.

Contribute to continuous improvement and the development of services for people with learning disabilities.

Qualifications and Experience:

• To be successful in this role you must have; Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
