About the job
We're seeking an exceptional Behavioral Health Case Manager to join our Enhanced Care Management team. Under the direction of the ECM Clinical Manager, the Behavioral Health Case Manager (internally titled Clinical Lead Care Manager (CLCM)) serves as the client’s primary point of contact and works with all their providers such as doctors, specialists, pharmacists, social services providers, and others to make sure everyone is in agreement about the client’s needs and care.
