Come and be a part of our Integrated Health Center with locations in Mentor and Lake County. The Prescriber offers psychiatric assessment and medication management within the outpatient Youth and/or Adult Psychiatry setting in the Health Center Program. This position is responsible for individual services to patients assigned by Crossroads Health. This licensed provider will engage as a member of a collaborative team.
With a Trauma Informed approach, this position will be a representative for the organization and how we offer integrated services for recovery and mental health.
