LPN/RN Nurse Navigator – Palliative Care
Phoenix Home Care & Hospice is seeking a compassionate and experienced LPN or RN Nurse Navigator to join our Comprehensive Disease Management (CDM) / Palliative Care team in Springfield, MO and surrounding communities.
- Schedule: Full Time | Monday–Friday | 8:00 AM–5:00 PM
- Location: Springfield, MO and surrounding areas
- Salary: $60,000–$70,000 per year
