Responsibilities
Evaluate patients’ physical, cognitive, and functional abilities to identify areas that may affect their independence and quality of life
Develop individualized treatment plans based on each patient’s needs, goals, and abilities
Provide therapeutic interventions to improve patients’ ability to perform activities of daily living
Help patients develop and maintain skills needed for bathing, dressing, grooming, meal preparation, and other daily activities
Recommend and teach the use of adaptive equipment and techniques to promote independence and safety
Assess patients’ homes and recommend modifications to reduce fall risks and improve safety
Provide exercises and therapeutic activities to improve strength, coordination, balance, mobility, and functional independence
Educate patients and caregivers on treatment plans, safety techniques, and strategies for continued progress at home
Monitor patient progress and modify treatment plans as needed
Collaborate with physicians, nurses, physical therapists, speech therapists, and other healthcare professionals
Maintain accurate and timely documentation of patient evaluations, treatment plans, progress, and services provided
Promote patients’ independence, dignity, and quality of life through compassionate, patient-centered care
Follow all applicable professional, organizational, and regulatory standards
Requirements
Proven work experience as a Physical Therapist
Intensive education and clinical expertise
Ability to manage patients with different types of personalities
Current knowledge of treatment practices
Drive to continually learn and grow
Professional behavior
Excellent interpersonal communication skills
Valid license to practice physical therapy
Graduate degree or greater in physical therapy
