Remote Mental Health Case Manager (LCSW) - Mercy Virtual Collaborative Care

Join Mercy's Virtual Collaborative Care team and help patients and families navigate behavioral health challenges through compassionate, evidence-based care. As a Remote Mental Health Case Manager
, you will provide psychosocial assessments, therapy, crisis intervention, care coordination, and discharge planning while partnering with an interdisciplinary team to improve patient outcomes.
What You'll Do
- Conduct behavioral health assessments and develop individualized treatment plans.
- Provide individual, family, and crisis intervention services.
- Coordinate care with physicians, nursing staff, therapists, and community resources.
- Support patients and families with education, resource navigation, and discharge planning.
- Advocate for patients and encourage active participation in treatment goals.
- Monitor patient progress and adjust care plans as needed.
- Maintain accurate and timely clinical documentation in Epic.
- Collaborate with internal and external partners to ensure continuity of care.
Qualifications
- Master's degree in Social Work, Counseling, or a related human services field required.
- Licensed Clinical Social Worker (LCSW) required.
- 2-3 years of behavioral health experience preferred.
- Strong assessment, communication, crisis intervention, and care coordination skills.
- Proficiency with Microsoft Office and electronic medical records.

- Monday-Friday, 8:00 AM - 4:30 PM
- No weekends or on-call responsibilities
