Liquid Personnel is recruiting an Adult's
Social Worker to join its client’s
ASC Transformation team, based in Nottingham.
This is an exciting opportunity for a skilled practitioner to undertake complex reviews and reassessments across a wide range of Adult Social Care services, whilst contributing to service improvement and transformation initiatives.
Working across Older People, Physical Disabilities, Learning Disabilities, Mental Health, and Supported Living services, you will review support arrangements, assess changing needs and outcomes, and promote strengths-based, person-centred practice. Alongside frontline social work, you will play an important role in shaping new ways of working, improving processes, and supporting innovation within Adult Social Care.
What will your responsibilities be?
Undertaking complex reviews and reassessments across a range of Adult Social Care service areas.
