Bring Your Home Health Experience to a Consistent Residential Setting
The Arc of Ocean County, a nonprofit organization supporting individuals with intellectual and developmental disabilities, is seeking a compassionate and dependable Certified Home Health Aide (CHHA) to join our Residential Services team in Brick, New Jersey.
In this full-time position, you'll provide hands-on support to individuals living in a medical group home, working alongside residential leadership and nursing professionals to help meet both everyday and health-related needs.
This is an opportunity to use your CHHA training in a setting where you'll do more than provide personal care. You'll help individuals access necessary medical services, follow treatment plans, develop daily living skills, participate in their homes and communities, and live as independently as possible.
Help ensure individuals receive necessary health and medical services, including dental, psychological, and pharmaceutical services.
Administer medications as required and follow established medication procedures.
Assist with monitoring and providing clinical care under the direction of the LPN Medical Manager, Agency RN, and/or Residence Manager.
Follow individualized treatment plans, including medication regimens, dietary orders, behavioral plans, and the use of adaptive equipment when applicable.
Observe changes in individual health or well-being and communicate concerns to appropriate residential and nursing team members.
Provide Hands-On Daily Care
Assist individuals with activities of daily living, including personal hygiene, grooming, cooking, cleaning, laundry, shopping, and other everyday needs.
Provide direct support and supervision while encouraging individuals to complete tasks as independently as possible.
Help individuals develop practical daily living, personal, and social skills.
Provide support in a manner that respects individual dignity, privacy, rights, and personal choice.
Encourage Greater Independence
Become familiar with each individual's Individual Habilitation Plan and provide support consistent with identified needs, goals, and objectives.
Teach and reinforce skills that can help individuals become more independent in their homes and communities.
Encourage participation in recreational and social activities.
Support personal and social development within the least restrictive environment possible.
Help Maintain a Safe & Comfortable Home
Contribute to a clean, safe, secure, accessible, and welcoming medical group home environment.
Complete assigned household responsibilities and help maintain the condition of the residence.
Identify and report maintenance or repair needs promptly.
Follow agency procedures involving health, safety, confidentiality, individual care, and residential services.
Communicate & Document With Accuracy
Complete required daily, medical, training, communication, and program documentation accurately and within established timelines.
Document progress toward individual goals and objectives.
Report incidents promptly, accurately, and comprehensively.
Communicate professionally with supervisors, nursing staff, families, guardians, healthcare professionals, and others involved in supporting each individual.
Use Your CHHA Skills in a Different Kind of Care Environment
Working in a medical group home gives you the opportunity to become familiar with the people you support—their routines, preferences, health needs, goals, and accomplishments.
You'll work as part of a consistent team while combining home health care, clinical assistance, direct support, and skill development within an individual's home.
The goal isn't simply to provide care. It's to provide the right level of support while encouraging each person to participate, make choices, develop skills, and maintain as much independence as possible.
