Overview
Under the general supervision of the Associate Director, or designee, the Behavioral Health Care Partner (BHCP) Clinical Coordinator (Team Lead) is responsible to develop, prioritize, and oversee staff activities that support completing and managing a caseload of nursing facility residents who have been identified as program participants through the Preadmission Screening and Resident Review (PASRR).
This team ensures that nursing facility residents with confirmed serious mental illness (SMI) receive all recommended PASRR services and supports, and, for those residents who are interested, that progress toward community living is monitored. This position ensures that Care Coordinators complete person-centered assessments, service planning, and ongoing follow-up for these residents, including coordination of specialized services and additional behavioral health services.
