Our client based in Walsall is seeking an experienced Social Worker to join their West Locality Team. This is an excellent opportunity for a dedicated practitioner to support adults with care and support needs, along with their carers, to achieve positive outcomes, maximise independence, and improve their overall wellbeing through strengths-based and person-centred social work practice.
The successful candidate will manage a varied caseload, undertaking assessments, care planning, safeguarding enquiries, and reviews whilst working collaboratively with individuals, families, carers, and partner agencies. The role requires a proactive practitioner who can work autonomously, manage risk effectively, and deliver high-quality interventions in accordance with the Care Act 2014, Social Work England standards, and local policies and procedures.
What will your responsibilities be?
Undertake strengths-based assessments for adults and carers in accordance with the Care Act 2014.
Develop, implement, and review person-centred Care and Support Plans that promote independence and positive outcomes.
Manage a varied caseload, ensuring assessments, interventions, and reviews are completed within statutory and local timescales.
Identify, assess, and effectively manage risk to safeguard vulnerable adults.
Lead safeguarding enquiries, safeguarding planning, and reviews to protect adults from abuse, neglect, and exploitation.
Complete Mental Capacity Assessments and contribute to Best Interest decision-making where required.
Advocate on behalf of adults with care and support needs, ensuring their voices are heard and access to advocacy services is provided when necessary.
Work collaboratively with families, carers, health professionals, community organisations, and partner agencies.
Promote strengths-based practice that empowers individuals to utilise their own skills, support networks, and community resources.
Maintain accurate, timely, and high-quality case records, assessments, reports, and support plans.
Participate in supervision and escalate significant concerns and risks appropriately.
Support, mentor, and share knowledge with students, newly qualified social workers, and less experienced colleagues.
