About this Role
Serve as a critical clinical connection point across Home Health, Hospice, and Physician Services, ensuring patients receive timely, coordinated, and high-quality care. As a Triage Registered Nurse, you'll respond to patient needs as they arise, guiding care decisions, coordinating services, and facilitating clear communication between patients, families, providers, and care teams throughout the healthcare journey.
As a key clinical resource, you'll help ensure every patient receives the right level of care at the right time while supporting continuity, safety, and positive outcomes.
This role is ideal for an experienced RN who is confident in clinical decision-making, thrives in a fast-paced environment, and excels at balancing urgency with compassionate care. You'll assess patient needs, coordinate care plans, provide after-hours clinical leadership, and support field staff to ensure safe, effective care delivery across all service lines. You'll also contribute to staff education and onboarding by sharing clinical expertise and reinforcing best practices.
