Assistant Program Director – Care Coordination
The Assistant Program Director will work closely with the Director to oversee and support the operation and development of the Care Coordination Program ensuring services are of the highest quality, and in compliance with policies and procedures developed by the Lead Health Home, Community Health Connections, (CHC), Department of Health (DOH), Office of Mental Health (OMH), and Mohawk Opportunities. Duties include:
- Assist with the growth and development of Care Coordination Services as the landscape of healthcare changes.
